PTA Forms

• Student Information & Image Consent Form: Barksdale PTA must have written consent to include your child’s name and/or photograph in Barksdale PTA publications.
• Formulario de consentimiento sobre datos e imágenes estudiantiles (pdf): Nuestra sección de PTA produce una variedad de publicaciones y proyectos con el fin de alentar un espíritu e interacción comunitaria y de reconocer a los alumnos y sus logros.
• PTA Plan of Work (Word)
• Check Reimbursement, Deposit, Tax and other Finance Form:
  • Check Request Instructions (pdf): Instructions on how to fill out a check request.
  • Check Request Form (Excel): Print and fill in all lines with complete and accurate information. You can fill the form within MS Excel or print it out.
  • Tax Exemption Certificate (pdf): Texas Sales and Use Tax Exemption Certificate.
  • Check Record (Excel): Use this form to account for all the checks received.
  • PTA Deposit Form (pdf): Use this form when counting funds and turning them in to the treasurer.
  • Check Request Form for Field Trips (Excel): Teachers use these to get reimbursed for PTA-funded field trip transactions.
  • Check Request Form for School Supplies (Excel): Teachers use these to get reimbursed for PTA-funded school supplies.

• Theme Search Form – Please mail to Texas PTA by November 5, 2011
• Student Entry Form – Please complete the bottom portion of the form prior to printing.
• Consent Form – Required when an individual’s recognizable voice or image is used in a Reflections entry.
